SQL Query based custom Monitoring Setup

Hello Team,

We are trying to setup the custom SQL query based monitoring via SCOM. e.g. we are looking to have a SQL query run against one of the Database of an application Database and return will be an integar value. Based on the result we will be using it to set threshold and trigger alert.

I did lot of search got some script online however confuse how we can use it. Can anyone please share some insight and if possible steps it will be really a gr8 help.
